Supervision Forms
Individuals under supervision may not leave the jurisdiction (State of Oregon) without permission of the Court or Probation Officer. Clients who need to travel outside of the state should request permission to travel as early as possible. Requests should be in writing and include details like destination, length of stay, and purpose of trip. Because a client's ability to travel depends on permission from the Court of Probation Officer, a client must not purchase a ticket for out-of-district travel without first obtaining an authorization.
Persons under supervision of the U.S. Probation Office may report misconduct by notifying a supervising probation officer at the numbers listed below or by calling the Deputy Chief U.S. Probation Officer at (503) 326-8600.
